In the United States, individuals who operate commercial motor vehicles (CMVs) must undergo a DOT (Department of Transportation) physical exam. This exam is mandated by the Federal Motor Carrier Safety Administration (FMCSA) to ensure that drivers are physically, mentally, and emotionally fit to perform their duties safely. Drivers who carry dangerous materials, transport more than 15 passengers, get paid to drive a vehicle that can carry over 8 people, or operate a vehicle that has a gross combined weight of 10,000 pounds or more are required to receive regular DOT physicals.
During a DOT physical, a certified medical examiner (CME) will conduct several evaluations:
• Medical history review: The examiner will discuss your medical history, including any past surgeries, ongoing medical conditions, and medications you are currently taking.
• Vital signs check: Your blood pressure, pulse rate, and temperature will be taken to assess your general health and wellness.
• Physical examination: The doctor will examine various aspects of your physical health, including your heart, lungs, abdomen, spine, and extremities. In addition, there will be detailed vision and hearing exams during your DOT physical.
• Urinalysis: This portion of the DOT physical checks for underlying medical conditions, such as diabetes.
• Sleep apnea test: If you have symptoms such as daytime drowsiness or heavy snoring, you may be referred for a sleep apnea test, although this is not mandatory for all drivers.
Under normal circumstances, CMV drivers need to undergo a DOT physical every 24 months. However, certain medical conditions may necessitate more frequent examinations. For instance, drivers with conditions such as high blood pressure, heart disease, diabetes, or sleep disorders may need to undergo DOT physicals more frequently to monitor their health and ensure they can safely operate a CMV. The frequency of these exams can be every 12 months, 6 months, or even 3 months, depending on the severity of the condition.
If you have a chronic illness, you can still pass the DOT physical if your condition is well managed. Drivers with controlled heart conditions can often pass the exam, but those with recent heart attacks, chest pain, or other severe cardiovascular issues will require additional documentation and clearance from a cardiologist.
Similarly, drivers with insulin-treated diabetes are no longer automatically disqualified. Drivers must provide an ITDM Assessment Form from their treating clinician to demonstrate that their condition is well-controlled. In addition, drivers with epilepsy or a history of seizures may be eligible for an exemption if they have been seizure-free for a specified period and provide detailed medical documentation.
